I had an orchiectomy in Portland with Dr. Robert Barham in November 2002, and I felt I should write about the great physical pain I endured during the operation. I believe this is something that people considering this surgery should be aware of, and so far I have not heard of anyone who experienced such pain during this procedure.

I am a transsexual woman in my mid-20s who has been living full-time for just over 4 years. I am not sure when I will be able to have SRS, so I saw orchiectomy as a good preliminary step.

Dr. Barham is a very qualified and experienced urologist, reasonably priced, and has had excellent reviews; these are some of the reasons I chose him. While I would highly recommend him for this procedure, I feel that his current approach to pain management was severely lacking, at least in my case. Dr. Barham performs orchiectomy with local anesthesia (Novocaine shots). While this may be enough for some people, I found it inadequate.

Most people I have actually talked to, and in reviews I have read, said they felt just slight pressure, things moving around, little pricks, or hardly anything at all. So I was expecting something similar, and didn't see any reason for concern. But I found the Novocaine shots into the spermatic cords to be very painful. I did NOT feel numb in my groin area after these shots, as some people have described. I mentioned this to Dr. Barham, and he assured me that I would be numb enough during surgery. Well, I wasn't numb enough because I experienced considerable pain throughout the entire operation, which took about an hour.

The pain was excruciating, and caused me to scream multiple times, with watering eyes and hands clutched to the surgery table. I don't know how I got through it, looking back. Once I was cut open—and the incision in the scrotum was also painful—there was little I could do, and I felt pretty helpless. It’s not like one can just get up and walk away.

Although it was obvious that I was in great pain, Dr. Barham didn't give me more Novocaine until I asked him, not once, but twice. Surprisingly, even then I was still in tremendous pain. If I had known just how excruciatingly painful this surgery could be, I would have asked about general anesthesia or about an epidural anesthetic, if either were available. I never in inquired about this, though.

Surgically, Dr. Barham knows what he is doing, hands down: Let there be no doubt about that. But if you go to him, make sure there is a CLEAR understanding that you don't want pain during the procedure. Even if my case was not necessarily typical, this surgery does have the potential to be very, very painful, and I found out the hard way. If you feel you haven't been given you enough anesthesia for any reason, don't be afraid to get bossy and demand more.

On the bright side, Dr. Barham did do an excellent job surgically, and my recovery has been uneventful, with very little post operative swelling. I was given a few days worth of Cipro to ward off any infection, and so far Vicodin has been enough to deal with any post-surgery soreness. Finally, I am very glad to know that testosterone and its damaging affects have been eliminated from my body.
